Short answer
In the form you choose three exam cities, in order of priority, from the list in the bulletin. Your choices are limited to the state of your permanent address or present address. The city choice cannot be changed later except when the window for correction in all fields opens, and NTA does not entertain requests to change the allotted centre.
Based on the NEET (UG) 2026 Information Bulletin. The 2027 bulletin hasn't been published yet — rules can change, so check it when it is.

The rule, explained
The cities offered to you depend on the permanent and present addresses you fill in. In some cases only one or two options may be available.
NTA advises choosing your own city or a neighbouring city in that state, and avoiding distant cities.
Centres are allotted by computer, mostly in one of your preferred cities, with no human intervention. But NTA can allot a centre in a city outside your choices, and it may merge cities where too few candidates opt for them. You may have to travel to another city.
Your exam centre is shown on your Admit Card. Requests to change the exam centre, date, time or medium shown on it are not considered.
No TA/DA (travel or daily allowance) is paid to any candidate, including SC/ST/PwBD/PwD candidates.
