The NEET (UG) form is filled online only, at https://neet.nta.nic.in/, in three steps: register and note your Application Number, complete the form and upload your images and documents, then pay the fee online. Your application is submitted only when the Confirmation Page is generated after successful payment.

The rule, explained
Before you start, read the Information Bulletin and check your eligibility. Keep ready your parents' and your own names with correct spellings, an ID (such as Aadhaar, passport or PAN), your date of birth certificate, Class X and XII mark sheets, address proof, mobile number and email ID. Your mobile number and email ID are verified by OTP.
Step 1, Registration: fill in your details, create a password (8 to 13 characters) and choose a security question. An Application Number is then generated. Note it down; you will use it to log in and for all future correspondence.
Step 2, Complete the form: log in with your Application Number and password. Fill in personal details, choose the question paper medium and exam cities, enter your educational qualifications, capture a live photograph and upload your scanned images and documents. Choose the medium carefully, because it cannot be changed later.
Step 3, Fee payment: pay by debit card, credit card, UPI or net banking. Bank processing charges and GST are extra. After successful payment, print the Confirmation Page. The fee is not refunded once paid.
Submit only one application. Multiple forms from the same candidate are not accepted and lead to cancellation of the candidature/result.
